„Taking Rosenfeld’s first psychoanalytic paper as a starting point the author suggests that his technique was rooted in trying to make the patient aware by verbal interpretation of his psychic reality as he experienced it in the session with his analyst. Four of Rosenfeld’s theoretical contributions for helping clearly to conceptualize the patient’s psychic reality are then outlined: his ideas about narcissism, projective identification, the role of history and external reality and his optimistic insistence that no matter how different it may seem the patient is always trying to communicate his predicament.“

„This ‘mad’ self-idealisation is a defence against the separateness of the ‘good’ object that arouses unmanageable destructive urges, like the envious spoiling of the good breast-mother as a source of life, and the primitive anxieties that ensue such as confusion, fragmentation and disintegration. Narcissism also takes the shape of the idealisation of the ‘bad’ parts of the self, often represented as a mafia gang, which are turned to as a source of superior strength and become mobilised in an organised attack against the dependent self and its sane object relations (1971a). Such narcissistic organisations create a split in the self and keep the sane self stunted in its development.“

„In diesem Band hat der Autor 13 seiner wichtigsten Publikationen aus den Jahren 1947–1964 neu zusammengestellt und einem breiteren Publikum zugänglich gemacht. Es handelt sich zur Hauptsache um Arbeiten zur psychoanalytischen Erforschung und Behandlung akuter und chronischer Schizophrenie, ein Gebiet, auf welchem der Autor Pionierarbeit geleistet hat. Entgegen der bis dahin gültigen Lehrmeinung, wonach eine psychoanalytische Therapie bei psychotischen Patienten wegen deren narzisstischer Beziehungsunfähigkeit nicht in Frage komme, konnte Rosenfeld an Hand überzeugender Fallbeispiele belegen, dass schizophrene Patienten sehr wohl imstande sind, eine emotionale Beziehung zum Therapeuten aufzunehmen, und zwar eine sehr archaische Beziehungsform, deren Phänomenologie und Psychodynamik, deren unbewusste Gefühls- und Phantasiedimensionen der Autor in teilweise faszinierenden Schilderungen umreißt und u. a. mit Hilfe von Melanie Kleins Konzept der projektiven Identifizierung zu erhellen versucht. Die Überzeugungskraft dieser Arbeiten beruht wesentlich auf der stringenten Verknüpfung von theoretischer Konzeptualisierung und klinischer Beobachtung sowie auf der Konsequenz – man könnte sagen: der produktiven Einseitigkeit –, mit welcher der Autor sich vermittels der psychoanalytischen Methode ganz auf die Erforschung und Interpretation der inneren Erlebniswelt psychotischer Patienten konzentriert. Insofern handelt es sich um eine wichtige Ergänzung zu den familientherapeutischen Ansätzen der Schizophrenieforschung und -behandlung, in denen überwiegend die äußeren Interaktionen zwischen den Kranken und ihrer Umwelt untersucht worden sind.
Weiterhin enthält dieser Band u. a. zwei umfassende Artikel über Suchtprobleme aus psychoanalytischer Sicht, eine sehr originelle Arbeit über die Psychopathologie der Hypochondrie, ein Kapitel über die Wechselbeziehungen zwischen Homosexualität, Paranoia und Narzissmus sowie einen Aufsatz »Zur Psychopathologie des Narzißmus aus klinischer Sicht«, der die Diskussion über den Narzissmusbegriff und die Auffassung und Behandlung narzisstischer Persönlichkeitsstörungen entscheidend mitgeprägt hat.“
